Pursuant to the Act of May 16, 2019, amending the Act on Patient Rights and the Patient Rights Ombudsman, the Healthcare Complex in Bolesławiec does not charge fees for additional nursing care provided to minors or patients with a certificate of significant disability.
Issuance of Opinions Regarding Medical Coverage for Mass Events.
Pursuant to Order No. 33 of May 25, 2017, issued by the Director of the Healthcare Center in Bolesławiec:
The Healthcare Center in Bolesławiec issues opinions regarding medical coverage for mass events.
An application for an opinion must be submitted to the Secretariat of the Healthcare Complex in Bolesławiec no later than 30 days before the scheduled start date of the mass event.
Opinions are issued within 14 days of receipt of a complete application.
The Bolesławiec Healthcare Center charges a fee of 100.00 PLN (gross) for issuing an opinion.
The application submitted by the event organizer for an opinion should include: information about the organizer (exact name, address, phone number), the first and last name of the responsible person along with a contact phone number, the date of the event, the location of the event, the expected number of participants, any additional information the organizer deems important for the issuance of the opinion, the name and address of the entity that will provide security for the event, and proof of payment of the fee for issuing the opinion. The application should also include the information specified in the Act of March 20, 2009, on the Safety of Mass Events.
An event organizer applying for an opinion may pay the fee directly at the ZOZ Cashier’s Office in Bolesławiec daily from Monday through Friday between 7:30 a.m. and 2:30 p.m. or by bank transfer to: Zespół Opieki Zdrowotnej w Bolesławcu, 59 – 700 Bolesławiec, 4 Jeleniogórska St., bank account: 91 1560 0013 2640 9151 4000 0001, reference: “issuance of an opinion regarding the security of a mass event.”
